This step comprises several components. They are:
- Identifying a patient in your practice to whom the problem applies and you think would be receptive to a new strategy.
- Printing the Patient Information Sheet and the Physician-Patient Partnership Paper (PPPP) and presenting both to the patient. Encourage the patient to consider the recommendations. The use of the PPPP is optional. Many physicians find that using a patient-centred method without recording a formal contract with the patient is adequate. Printing the patient information sheet may be all that is required. Physicians who are concerned that evidence-based recommendations may not conform to practice in their community may find the written agreement helpful.
- On the patient’s return visit, complete the PPPP form with the patient and negotiate an agreement based on finding common ground that is best for the patient.
